Instructions:
Download and open the release statement in Adobe Reader (comes with most computers, but if you don’t have it, it can be downloaded for free here).
Click “Sign” in the upper right-hand corner.
Click “Add text,” then click on the appropriate lines to fill out the information.
On the signature line, click “Place signature.” You can choose any of the four options: typing your signature to have Adobe generate one for you, using your webcam to take a photo of your signature that Adobe will generate into text, drawing your signature using the mouse or uploading an image.
Once complete, save the form and email to allabouthebreakup@gmail.com.
If you would prefer to mail in a physical copy of the release statement, email us at allabouthebreakup@gmail.com and we will get back to you with the address.
